The Remit:

The Community Health Partnership (CHP):

  • Ensure patients, carers and a full range of health professionals are involved. 
  • Establish a substantive partnership with local authority services (e.g. social work, housing, education and regeneration). 
  • Have a greater responsibility and influence in the deployment of NHS Board resources. 
  • Play a central role in service redesign locally. 
  • Focus on integrating primary and specialist health services at local level. 
  • Play a pivotal role in delivering health improvement.

The Forum:

The CHP Committee normally meets four times a year.  Only three meetings were held during 2007/08 due to local government elections in May 2007:

  • 6th September 2007
  • 22nd November 2008
  • 30th January 2008

Forum Activity:

  • Three CHP Committee meetings (as above).
  • One seminar on "How Do Councillors Improve Health and Community Wellbeing?" which took place on the 13th May 2008.
