The Remit:
The Community Health Partnership (CHP):
- Ensure patients, carers and a full range of health professionals are involved.
- Establish a substantive partnership with local authority services (e.g. social work, housing, education and regeneration).
- Have a greater responsibility and influence in the deployment of NHS Board resources.
- Play a central role in service redesign locally.
- Focus on integrating primary and specialist health services at local level.
- Play a pivotal role in delivering health improvement.
The Forum:
The CHP Committee normally meets four times a year. Only three meetings were held during 2007/08 due to local government elections in May 2007:
- 6th September 2007
- 22nd November 2008
- 30th January 2008
Forum Activity:
- Three CHP Committee meetings (as above).
- One seminar on "How Do Councillors Improve Health and Community Wellbeing?" which took place on the 13th May 2008.
