Aberdeen City Community Health Partnership
Community Health Partnerships (CHPs) were established in April 2004 to build on the achievements of Local Health Care Co-operatives (LHCCs) and to make a direct and measurable improvement in local population health and to provide higher quality, accessible and joined up services to local communities.
CHPs are a key building block in the modernisation of NHS and joint service, with a vital role in partnership integration and service design.
Partnership for Care states that CHPs have a new and more consistent and enhanced role in service planning and delivery working as part of decentralised but integrated health and social care systems.
Community Health Partnerships:
- Ensure patients, carers and full range of health professionals are involved;
- Establish a substantive partnership with local authority services (e.g. social work, housing, education and regeneration);
- Have a greater responsibility and influence in the deployment of NHS Board resources;
- Play a central role in service redesign locally;
- Focus on integrating primary and specialist health services at local level;
- Play a pivotal role in delivering health improvement
